Stock Gift Sample Letter

To execute an electronic transfer of your stock to Rose Brooks Center, please transfer stock to the Rose Brooks Center account at Midwest Trust Company by completing a gift letter and sending it to your brokerage firm. Transfer instructions are on the letter.

The date of your gift is the date that Rose Brooks Center receives the shares in its account. The value of the gift will be the average of the high and low prices on the day the gift was received into Rose Brooks Center’s account. You will receive written confirmation of the value and date of your gift for income tax purposes.

Refund Policy
This is the Donation Refund Policy for Rose Brooks Center. Rose Brooks Center collects donations on its website to support our mission. If you believe that an error has been made in connection with your online donation, please contact katym@rosebrooks.org. We will endeavor to work with you to correct any such error, and if necessary, make any refund or correction that is appropriate. If you change your mind about donating online to Rose Brooks Center, we are happy to honor your decision as long as you request a refund (return of donation) with 5 business days of the date on which you made the contribution.
